Lake Champion ‘09 Registration
This year Registration will be online. Please fill out the form below and when you are done hit the Submit button. You will then receive a confirmation email in your inbox. Be sure to print the Release Form for your Students and have their parents/guardians fill them out. Remember for Insurance purposes please bring a copy for yourself and a copy to give to me when you check in for each student in case of an emergency.
